Welcome to our activity release page. To reduce paperwork and
increase convenience we have gone to a 12 month activity release. If
you have any questions about this, please contact us.
Please complete the form for each child in our program. When you click
on the submit button it will be emailed to a Trinity Lutheran Youth
Ministry staff member. We will email you to confirm that you sent the
release. Please take time to read our liability form posted here. Thanks
